Our Residential Habilitation and Support Services provide up to a full day (24-hours) of services and/or supports which are designed to ensure the health, safety and welfare of the participant, and assist in the acquisition, improvement, and retention of skills necessary to enable them to live successfully in their own homes.

Services are provided outside the home and are designed to support learning and assistance in the areas of self-care, sensory/motor development, socialization, daily living skills, communication, community living, and social skills. Community based activities are intended to build relationships and natural supports throughout the community in which your loved one lives.

Respite Care services are a short-term care solution for individuals unable to care for themselves. Respite Care provides temporary relief to those unpaid persons (such as a parent or guardian) normally providing care. We offer care within a variety of settings such as the consumer’s place of residence, the caregiver’s home, DDRS approved day habilitation facilities, and in non-private residential setting (such as a respite home).

The quality of care our consumers receive is our number one priority. Through our Family and Caregiver Training, a parent, family member, or primary caregiver has the opportunity to learn more about the treatment regimens and use of equipment specified in their loved one’s Individualized Support Plan (ISP).

Our Supported Employment Follow Along program consists of time-limited supports (18 months per employment setting), that enable our consumers to maintain employment in a competitive community employment setting. Participants must be paid at or above the federal minimum wage in order to qualify.

Workplace Assistance Services offer on-the-job personal care supports to consumers working within a competitive community employment setting. These supports are designed to increase the retention of consumers in the workplace by ensuring their health, safety and welfare while they work.

We offer Transportation Services to waiver participants, enabling them to gain access to non-medical community services and resources, maintain or improve their mobility within the community, increase independence and community participation, and prevent institutionalization as specified by the Individualized Support Plan (ISP) and plan of care.

Community Transition Services include reasonable, one-time set-up expenses for those making the transition from an institutional setting (such as an Intermediate Care Facility) to their own home in the community. Community Transition Services are not reimbursable on any subsequent move.

An option under the Family Supports Waiver, PAC Services are designed to enable consumers to remain and live successfully in their own homes, function and participate in their communities, and avoid institutionalization. PAC services support the participant in activities of daily living, self-care, and mobility with the hands-on assistance, prompting, reminders, supervision and monitoring needed to ensure their health, safety, and welfare. Activities may include any task or tasks of direct benefit to the consumer that would generally be performed independently by persons without developmental disabilities or by family members for or on behalf of their loved ones with developmental disabilities
